Parent Survey Form

Parents can provide important feedback to their children's teachers if they are given a forum to do so. I send out a questionnaire to my students' parents each quarter of the school year to gain their perspective on their children's participation in my program. What I have found is that I hear more frequently from those who are quite happy with what is going on at school. I have also been able to use the information I have gained to show the high level of parental support for my program to my administrators and to our district's school board (this has been very helpful in dealing with curriculum and other program issues).

Below is a copy of the survey I use.


 

How am I doing?

As a teacher of a multiage program at Chimacum Elementary I want to be sure that I am doing the best that I can. I am regularly receiving feedback from the students, from other teachers, and from the principal. I feel that it is important to get feedback from my students' parents as well. Please help me by completing a copy of the following form.


In the first section, check the box in the 1 column if you feel that my performance in the various areas is well below your expectations. Mark a 2 if you feel I could improve, a 3 if I am meeting your expectations, and a 4 if my performance has exceeded your expectations. If you don't know please check the column labeled 0 .
